I know it because I deal with them on a daily basis and get a daily statistical report summarizing claims submitted, accepted and denied by all of my customers. That's how *I* know.

How can a Pigfcker know? You can find out by consulting the following chart, which is an empirical survey of claims actually submitted, according to the AMA 2008 National Health Insurer Report card.
