But, clearly, it ISN'T the "only one" since --- as I've already pointed out --- that's the STANDARD FORM in ALL IT'S PARTICULARS that the state of Hawaii issues now. That's the ONLY BIRTH CERTIFICATE they issue now! (And there are several other states issuing similar forms, no big deal....)
