That doesn't seem like a "medical error" to me. Shit happens.

A girl I once knew well was given the wrong drug in the hospital and it killed her -- this was about 25-30 years ago. THAT was a medical error -- can't remember details but it was blatant. This kind of thing is, I think, relatively infrequent these days -- although mistakes happen. In any business, including health care.
