Improving the overall wellness of folks with one foot in the grave? LOL. The only way you get more than one option is to break rationing down into multiple variations. You could use HMOs rather than fee for service, reduce coverage, engineer waiting lists, or increase co-insurance/co-payments. It still comes down to rationing.
