the comparison gets very complex because we are talking about decades with multiple countries, and all different sorts of mechanisms to control costs.

I agree that there is no real way to compare. But it stands to reason that if some things are rationed, costs will be more than if nothing were rationed but less than if all things were rationed. You're not disputing that rationing reduces costs to the payer, are you?
