Governments and Insurance cos don't operate on that basis. We do, but we don't pay the bills.

Actually from a net present value perspective, the same "dollar expense" of end of life 30 years from now is a lot less than end of life expenses 10 years from now.

But its probably not the same dollar expense, its a larger one. You pay the extra for the preventive care for those 30 years. Also, to the extent it makes people live longer, you pay for ordinary treatment for the extra years.
