Limbaugh was not an employee in Hawaii. He was a tourist who brought with him his own insurance coverage.

Delivered by what is essentially a socialized health care system run by people in unions...which is what he admitted to.

Wrong, fortunately it will still be available at private hospitals in other countries.

While US heart surgery expertise is not easy to match, most if not all besides that is available at same or sometimes even better consistent quality for those who can pay.

And it isn't that expensive, actually...
In Denmark and Finland, just to mention 2 places, or in Germany.
