A bit puzzling how they reconcile this with their "max allowed charges" bible, but somehow they do.

One would hope they have a small thread of libertarian in them (ha) as well as self interest.

OK, it's all self interest, I suppose. If old folks want to spend their own money for medical service, then it's no skin off of Medicare's nose. If the patients were using ordinary services, Medicare would have foot the bill, however niggardly. When the patient goes outside the system, the patient pays. That's good for the bottom line.

In this case maybe the powers that be are prepared to slide a bit on the ideology in favor of the bottom line, particularly since they would assume that those who pursue such services are rich and thus targets for soaking.
