I have a private communication from someone with experience in a data integration effort in one single hospital. Everybody involved felt threatened by it and refused to cooperate, preferring to defend their own turf. Finally management gave up and developed separate systems for each department, which they then integrated after the fact. Took twelve years.

This is pretty typical.

I blame it on management. Management does not have a clue of what the problem is because they've skipped right over it in their rise to the top. Therefore, they do not know how to marshal support from their staffs. They don't know enough to know when they're being told a crock. They don't know when an intractable disagreement needs a hard decision, even if it's not optimal.

It takes a knowledgeable leader, one with guts AND knowledge of the problem of integration. It's very hard to find such a person in leadership roles.

I value leadership, and have seen darn little of it in my experience. I know enough about it to know that I'm not a good leader, but I know one when I see one.
