What makes you say this? I don't think state regulations dictate what kind of policies are sold. I think the insurers themselves design the benefit packages they want to sell.

But the insurance companies aren't in business to cut costs overall. They're in business to make a profit. And they make a lot more profit on frillier policies.

What would motivate them to seek the lowest level for basic coverage, where they make almost no money? Why would they bother to enter new markets in order to do that?
