Not just two differnt programs how their written, but two totally different ideas. Sure you could put one function under another program. You could also have the DoD enforce federal law, or have NASA run hospitals for the poor, but there isn't any good reason to set it up that way. If you want a national subsidized disability insurance program (either in addition to Social Security Disability, or instead of it, or an expansion of and improvement of that system), it seems to me to make a lot more sense to call for that (or an expansion of unemployment insurance for people unemployed do to illness or disability) than to give them gold or silver plated health insurance.

The idea I tossed out was to cover the expenses directly related to the catastrophic conditions. The regular annual expenses, unrelated to this conditions, shouldn't be significantly higher than anyone else's. If they have severe financial problems that might arguably justify extensive help being provided to them, its more likely to be because of the rent, or the car payment, or the mortgages, or credit card debts (for non-medical purposes), than it is to be from their ordinary medical bills. Why focus the help there?
