I think in a situation when most purchasers are national governments (or fully owned "government subsidiaries"), it is no longer a "free market". All those decisions - how much to pay for meds, etc - are already being made on the level of government bureaucrats.

There are a lot of industries where the primary customers are governments. Just very few (if any) where a government is stupid enough to make it illegal for themselves to negotiate.
