If they could do that now, they would. To the extent the governments have monopsony buying, or even in a few cases explicitly price controls, or at least heavy political pressure and control on prices, the drug companies have little pricing power with other countries. They could try a take it or leave it approach, but even that might not work, as if other countries are cut off they may set up a compulsory licensing system and give the rights to their local companies to produce drugs who's patent is held by American companies (or potentially companies from other countries that want to offer the same "take it or leave it" price to the governments)

No they haven't. Governments have organized them in certain ways, that's a very different thing.
