No different than extending all kinds of "entitlements" to a growing population like education, fire services, police protection, food safety, the common defense and on and on and on.

All those are taken for granted as government services we find a way to pay for. Only healthcare is considered a special "entitlement" in America, a concept most other countries would find completely absurd.

The problem is motivating change in a system where 90% of the players don't need the extra government protection. That's where the individual motivation undermines the macro view (if you believe that only covering 90% of the population is inherently bad, which many don't).
