Toyota screw up : I don't think this is an example of captured regulators as much as slow moving regulators.

I also think that the GROSS negligence came in the design part, when Toyota did NOT require the "Brake wins" Override that Nissan, Mercedes, and everybody else had.

Later at Toyota, there is the ordinary response to the acceleration problem as being first floor mats then brake pedals but not the electronics / software, which is enormously complicated.

That is analogous to some one who lost their car keys in the bedroom looking for them in the living room because the light is better. That is how MOST human organizations work.
Look at what NASA did for the first Space Shuttle accident.

The few well know examples that work differently are the aviation accident investigators, some research scientists, doctors on TV (House) and the experts who are brought in to fix things after the first and second groups could not fix something. These examples fit the Sherlock Holmes method, and they are the exception.
