claim processing and other low level admin work because it's likely cheaper It probably is cheaper, but not quite as cheap as people think. After all, they are the ones who do the actual real work. Distributing funds to carriers is not the same as actually running the program. How much it costs to have carriers do their work appears to be a well kept secret. Publishing those numbers would blow sky high the myth of Medicare having an (overall) overhead in the single digits.
