Re moral hazard and medical providers, you have two players, the providers and the patients. With fee-for-service, whatever services are ordered are paid for by the carrier at the going rate. Both the patients and the providers are incentivized to opt for more and more services since they cost the patient nothing or next to nothing and nicely line the provider's pockets. The patients ask for them, the providers order them, and the carrier pays for them.

What additional fee do you have in mind? In addition to what?
