If the provider were paid a flat fee for an entire procedure made up of a bunch of services, it would be in his best interest to limit the number of services (to save money)

"Fee for service" or "Fee for procedure" are essentially the same thing. The service provided is performing the procedures. You could break down "procedures" into parts you call "services" or "services" in to parts you call "procedures". A meaningful move away from "fee for service" would be "fee for patient", or "hourly pay", or some other method of payment, not to breaking down the different "procedures" and "services" in to different groupings.
