I don't know, I can't be sure it was intentional, but the intentions matter less than the actions. If you where trying to set up a system to underrank the US, while still wanting it to actually measure something and retain even the most minute amount of credibility, you would be hard pressed to do better. But since I can't actually back up the claim of intension, despite how much it looks like that was the case. I withdraw the claim. I can't read their minds, I can't be sure of their motivations. I can however be sure that they created a study with faulty methodology in terms of what it measures, if you goal is to actually reasonably measure the quality of health care in different countries.
