I had a sudden acceleration problem with a rented Toyota a few months ago. I was wearing large over-boots during a blizzard, so my extra-wide footgear was pressing both the gas and brake pedals at the same time. I quickly solved it by pulling back my foot and repositioning it.

Just to be extra careful, I went out today and practiced shifting my car into neutral while driving at 45 mph. It was easy and foolproof: simply pull the gearshift to the left and forward. From now on whenever I drive an unfamiliar car I'll make sure I know how to shift into neutral without taking my eyes off the road.
