2 - I'm skeptical about preventive care saving any money right now, and even if its better targeted in the future I'm skeptical about it being effective enough to be a major cost saver.

That's were we differ. Childhood (and adult) obesity, smoking, bad diet, lack of exercise all contribute to chronic disease. Things that cost a lot long term like diabetes, heart disease, emphysema, etc. Yes those people will get sick and die eventually anyway but during their lives, if they change their lifestyle, they will use less medical services and far fewer drugs.
