In a free competitive interstate market, as pushed by Coulter, there would be no need for companies to have group plans or self insure. Individual plans would be customized and economical. Corporations could still subsidize an employee's individual insurance if they chose, but they would not longer be responsible for providing the choices for each employee.

That a what came first chicken/egg problem. You could make all the changes Coulter rec'ed but corps are not going to risk losing employee's by unilaterally canceling their health benefit. And remember the heads of corps are actual people who don't want to lose their "free" benefit.

As part of the elimination of employer provided insurance you could mandate (I know you guys hate that word) that the employees receive a pay increase exactly equal to the cost of their former insurance plan. That would make it more palatable to the employees.

Walla... a free market of 100+ millions consumers ready willing and able to buy insurance. And insurance companies saying "Damn we don't get this right and we are toast. Get our providers on the line and tell them we need better prices!".
