The tax revenues are not included as part of the "under $1 trillion" cost for the first decade.

The real meaningful number is the cost per year, since this is going to be a permanent entitlement. That number is likely going to start at $200 billion and go up from there.

Postponing the benefits until 2014 does not lower the annual cost. But try and nail any Democrat down on the annual cost, and he or she will either play dumb or talk about the poor uninsured (who won't even get their benefits until four years from now).
