As I've previously posted here, by far the largest chunk of it comes from simply transferring what is ALREADY being spent in corporate welfare on subsidizing the Medicare Advantage plans over to this.... (But, there is no 'VAT' in this legislation, and actually big reductions in the federal deficit --- up to $1.2 TRILLION ---are projected in the second decade when all of the elements are in place....)
