The Medicare prescription drug benefit was more limited in scope and was paid for directly out of the federal budget.

Obamacare forces much of the costs onto health insurance providers in the form of regulations and requirements, some of which will be absolutely crushing to all but the most established (i.e. powerful) companies.

The "hidden costs" are not that well hidden. The reasons why many people don't go out and find the truth for themselves include:

a) The problem is too complex for many of them to grasp.

b) Most people have either already decided that we must insure everyone or that socialism is wrong. Not much legitimate debate can occur when both sides are boiling things down to sound bites.
