It seems that with private contracting the doctor must be either in - or out. Can't do consults under Medicare, and run the office practice outside the system.

I was aware of that. I had assumed that that cardiology practice was opting out of Medicare. The piece doesn't say but the move doesn't make sense otherwise.

That they are opting out may explain the wide range of fees. I assumed that the low end of the range was, in part, for transition for some current elderly patients who need only minor attention and couldn't pay more out of pocket.
