Yes i know we wait because we choose to wait ..

If that is really true that you "choose", I have no problem with it. I'd like to keep the choice to decide not to wait because the government decides I "need" to.

amazed at your in the clouds elitism compared to the rest of your fellow citizens.

OK. Now I'm starting to get a better idea of the basis for your attitude.

Most people in the US get the same health care that I do. I use both Medicare and Blue Cross. These are not premium packages. Each is widespread and standard, used by average Americans. They cover what they cover.

Yes, I have used boutique services now and again, for which I chose to pay out of pocket when I judged the extra service worth it. That's equivalent to Canadians choosing to pay out of pocket for health care in the US. I don't see why you consider that such a big deal. It doesn't seem elitist to me. Mrs. Trump probably doesn't bother with insurance like us average Americans. Now, that's elite. For almost everything I have gotten standard treatment using my standard coverage. It doesn't involve much waiting. My salivary gland event was entirely standard. I recently discovered a pulsatile bump in my neck. Turned out to be an anomalous carotid artery. Took a (very stressful) week to get the CT scan and get a diagnosis from a specialist. Standard, not boutique. Comes from having plenty of capacity. The capacity is there because business is booming and capacity is profitable. Econ 101.
