Somewhere out there is a driller's site with a very good breakdown of what happened. Includes an interview with a worker that just got off and was getting ready for bed when the well "burped"...i think that's what they called it. I guess the gas and oil tries to force its way up the well. In any case, it happened twice and he drillers' discussion seemed to imply that after the first bump, the switch for the BOP should have been deployed. The second burp was immediately followed by the explosion, apparently vaporizing both sites where the switches activating the BOP were located...along with 11 workers.

It was late when I was reading the synopsis so i could be wrong, but that is my recollection.
