All companies use them these days, and government departments in particular.

Paperwork can be helpful to fix and control things or it easily can be a weapon of mass destruction in the wrong hands.

Risk Assessment Forms should help to illuminate thinking, not be used as a weapon to dissolve responsibility and blame on some poor underling.

As said, I could not find the proper standard form on line.

It should have three columns. Severity, Frequency, Detectability. Different headings can be used but would mean the same thing.

OK, detectability can be dropped in some cases. Not many.

Google or search on the words "Risk Assessment Form"

hundreds of versions.

And that is a bad sign. Why keep dicking with it??
