I suspect that what's needed is better risk analysis. For example, after a shuttle reentry disaster NASA decided to keep a backup shuttle on the pad ready to launch if the orbiting Shuttle were unable to reenter.

In the case of deepwater drilling a BP executive stated that BP knew that the chances of a large blowout were very remote - but conceded that the environmental consequences of a blowout would be severe. BP had disaster plans - but 3 million ft of oil booms and a few skimmers obviously wasn't enough.

In retrospect there should have been a solid backup plan in place to implement within 24 hours of a blowout preventer failure. What this plan should be I don't know - but it shouldn't include building a containment dome AFTER the accident.

Finally, someone independent of BP should be approving the disaster plans.

That raises a whole bunch of other issues.

That nuclear power plant 20 miles away from me... exactly what are their disaster plans and does the NRC vett them?

Who's in charge when the next hurricane aims at a coastal city?

We seem to have disaster plans for small scale events like airline crashes or local floods... but the big ones? Well, we never know until the accident happens.
