Dr. Oliver: Thank you for the excellent report and follow-up. A few questions: 1) Are any other parameters being used besides CD4 counts and viral loads 2) Is a 3 drug combo treatment being recommended as initial treatment for asymptomatc HIV 3) Is the prognosis (morbidity/mortality) improved for those who respond to 3 drug regimens (ie., is aggressive initial therapy indicated and is the the patient's clinical status an important factor [ie., CDC class, Karnofsky score] 4) How often should CD4 counts and viral loads be done and are meds adjusted depending on results? Thanks in advance.

Maybe, I don't remember now. I really do not care very much since I will be using it minimally. And when I use it it will be in combination, and there the dosage changes. If I prescribe it it will be BID (2x daily), with food. Remember that SQV is the least potent of all the PIs. Keep it up John, JLL

Adherence: I think the NUMBER of pills is much less significant than the TIMING - though perhaps I'm used to it because I've taken 15-20 pills (vitamins) a day for years. Can Fortovase be taken with meals? Viracept can be taken with food, before a meal or after a meal, though AGPH has done some very small studies that indicate 40% better bioavailability with food because the drug needs proteins to bind to for transport. The dosing schedule for Crixivan is hell; at least 1 hour before a meal and/or no less than 2 hours after a meal.
