Of course, the best healthcare system in the world would have mistakes made within it. As all healthcare systems will. and the main problem dummy is the design of the equipment It seems human the design problem was the equipment simply not being differentiated in a way to lessen the possibility of human error. Since there's no reason to think the design is any different anywhere else in the world, the case you brought up is, as usual, irrelevant to the issue discussed on the thread.
