They position the idler pulley just above the hole to help keep tension in the cable and to centralize the cable inside of the casing.

If the idler pulley wasn't there, the cable would jump about as the speed varied. It would probably rub on the top edge of the casing, which would be undesirable. As it is, it doesn't look like the hole is completely vertical anyway, so the cable probably does drag someplace down the hole, but it doesn't do so with as much stress as it would dragging at the top of casing.
