"Who decides when the class inequalities are too large? Who decides what constitutes too large?"

If no one decides, capitalism will decide and the end result will be the same as fascism, communism, socialism, monarchy, dictatorship etc. The few will control the many.

"I see class inequality as a natural manifestation of the differences between people."

100% agreed. What matters is the size of the inequalities and whether or not they are stable.

"I would rather see the government enforcing greater competition by preventing monopolies and oligopolies."

That might actually work and may be all that is needed to keep the class inequalities from becoming so large as to become destabilizing to the society.
