Yes - it should be available and paid for by insurers - but never required as a provision of coverage.

Give one example of country on this planet where people are better off without a government that provides access to health care.

Everyone would of course be better off with every last ounce of possible care - but the question is how much care can a government afford. You can claim we'd be a better country if everyone went to college - but we can't afford that either.
