bits & pieces I've read suggests that on, probably the loop-filter of, the pll someone left the thick-oxide mask off; usually these transistors run at high(ish) voltage so they'd probably fail in a reasonably short amount time.
probably an IP integration snafu. should've been caught in stress testing.
