As NASA showed with the Challenger disaster, engineers are frequently overruled by spreadsheets and other terrible management practices. Recently, a retired Boeing engineer's presentation has resurfaced where he warned of the 787 disaster which would occur if the company chose the path it did. You can lead a horse to water, but you can't make him drink. Engineers also make plenty of mistakes, to be sure. Got Metric? (JPL Mars [crash] lander project...)
