You put forth MA as an example of commercial insurance "costing more" than Medicare. But this is idiotic; you can't compare them because managed care is ALWAYS more expensive because managed care delivers a radically different product.

Yes, but isn't the added benefit paid for by the fed subsidy, patient additional premiums and co-pays?
