Andrew, The two papers that came out in Science last week indicate that virus in resting cells don't have mutations. However, an upcoming PNAS paper indicates that in these patients, other cells are producing virus, and the replicating viruses are the ones that develop resistance. Resistance is principally cause by mistakes made when copying the virus by reverse transcriptase. If the virus isn't replicating, then there is no resistance, but there is no killing either because the drugs work on active viral enzymes.
