Insurance companies do not pay claims or deny them based on QALYs. And when they refuse to pay a claim that results in harm to someone, there is a legal process in place to deal with them. As a result, it doesn't happen with any frequency.

b) I really don't care -- I don't like the Republicans much more than I do the Democrats.
