Every decision has a cost that has to be balanced against other costs. Add to that fact that humans are not prescient, and can't see the true future cost of any action. There is a lead time for information. "Festering" may be simply there was not enough information to management to make a change of decision.

This is why there is such a thing as "the business judgment rule" in corporate law. Everyone always makes better judgments from the back seat with 20/20 rear-view vision.
