On indexes--I don't know what to recommend; seen a couple in a book store that I scanned through, but I did not find them particularly illuminating--may be someone else can suggest something. Besides McMillan's book deals with the subject of options trading very thoroughly--index options are the same concept except that one is dealing with a synthetic as opposed to a stock.

On futures--I have found "The New Commodity Trading Systems and Methods" by Kaufmann to be very good. Schwagger and Murphy also have good books on the subject.
