I thought that going back to the asylum was actually what Carl wanted, so there was no real sacrifice. It seemed to me he was ill suited to the nuances of the outside world and that he recognised this. That is why I did not think the movie had an unhappy ending. Carl, I think, was back where he felt he belonged. And the only thing that dissapointed me about the movie me was why the mother didn't kill the boyfriend herself, but JP is right, that brings up a whole host of other problems, and Carl wouldn't have got back into the asylum if the mother had killed the boyfriend herself.
