what i said was they have the ability to cost shift their losses in uncollectable accounts onto insurers and the insured

I had some lab work done. $400 and my insurance paid $360 and I paid $40. 8 months later I get a bill for $360. I circled where my insurance had paid the $360. So I got turned over to collection. Good thing about being in collection is you talk to a person. The insurance had asked for the money back so they could pay out of a different account and the lab had not rebilled them.

Stuff like this makes that voice in my head say load the gun.
