"so how do you work with that one?" Well, I think Medicare spending needs to be reigned in. It's costs WAY exceed it's funding, largely because it can't say "no". WR could probably make some suggestions here. I think that it could all be negotiated (if the (R) party was SANE - which it's NOT, at the moment). We'll cut X amount from Medicare, by disallowing certain procedures, if you allow X amount to be cut from the military, such as ending the un-needed F-35 program. And so on. We may need for the (R)'s to start badly LOSING some elections to get there, but it's still possible.
