Back in 1981, some 30 years ago, I used to work for a hospital outpatient medical clinic. They insisted on giving patients only one appointment time -- 9am. When I would show up at 9am, usually I had between 20 and 40 people waiting for me. I had 2 RNs, some secretaries, technicians, etc. Quite a few people. Soon I found out that Medicaid paid the hospital $180 per visit. If I would see the same patient next door, in my private office, where I shared one secretary with a few other docs, the same Medicaid would have paid me $9.

So, this crap is not new, it's just getting worse. And more expensive.
