How about as a moderate who sees room for both (science and religions) in our culture and discussions?. I know it doesn't fit your survey's choices, but maybe you could make another category? :)

Did the Great Spirit come down and create life, the first living cell, or was it a chance lightening bolt activating some primordial gruel, or is there even a difference? We can call that God; it works to describe the cause of an unknowable event.

After that, it was all evolution.
Same for the the Big Bang and the universe, which is evolving to this day.
