It would be compulsory, in the sense that its paid for by taxes, and taxes are compulsory. If its going to be done, but I could pick how, I'd make it government provided catastrophic health care insurance. But it could also be government provided health care insurance coverage for more than just catastrophic expenses. I'm not a fan of the idea, I was just pointing out that it wouldn't be found unconstitutional (at least not without an enormous shift in direction by the courts).
