Due to state regulation, most insurers only operate in one state, and they organize to comply with that state's regulations. The "national" insurers are actually holding companies with separate operating companies in each state where they work.
I don't know the history of how the non-profits were originally capitalised. Here in MA, 3 insurers cover about 90%. The biggest is Blue Cross Blue Shield MA. The other 2, Harvard Pilgrim Healthcare, and Tufts Health plan both started in the 70's and were in some way originally affiliated with medical schools. I beleive that they started out very small.
