If they are able bodied, let them work of go hungry. If they breed more than once, sterilize them and take away child number two for adoption by a responsible couple.

Who said responsibility has to be enforced by anyone, with regards to a citizen looking after his own medical needs and his own retirement needs? If a citizen chooses not to be responsible, that is his concern and not the government's concern, provided that he does not resort to crime. If he resorts to stealing from others to meet his needs, or becoming angry and murdering others because he is poor, then the government has a responsibility to stop him. What is so difficult to understand about this? That is how freedom works.
